Start an ACT Referral
Assertive Community Treatment is a person-centered, compressive community-based, practice that provides integrated treatment, rehabilitation and support to individuals diagnosed with thought disorders.
Assertive Community Treatment (ACT) provides integrated mental health services treatment for individuals who have been diagnosed with thought disorders. Services include psychiatry, therapy, nursing, substance use disorder treatment, vocational services, case management, and peer recovery services. Teams partner with the individuals served and their natural supports to build the skills needed to manage their illness while also reaching their life goals. Common things teams help with include finding and maintaining housing, obtaining competitive employment, building social networks, and re-establishing relationships. Teams meet with people in their homes or community location of their choosing and provide services seven days a week, with crisis response services available 24 hours per day.
RADIAS Health provides ACT services in Ramsey, Washington, Anoka, Hennepin, Chisago and Isanti Counties. Referral and contact information:

Start a Youth ACT Referral
Youth ACT (also known as Intensive Rehabilitative Mental Health Services) are services provided by a multidisciplinary team directed toward youth ages 14-20 with a serious mental illness or co-occurring mental illness and substance use disorder.
These community-based services are designed to reduce hospitalizations for youth and improve their mental health, housing situations, and family interactions.
Services included are Diagnostic Assessment, Substance Use Services, Psychiatric Services, Skill-building for vocational and educational needs, individual and group therapy, crisis management, and family education and support.
Referrals may be made by school staff, family members, community partners, health care providers, and social workers. Youth may also self-refer.

RADIAS Health provides Intensive Residential Treatment Services in three locations, and we provide Crisis Residential Treatment in a unique collaboration with Hennepin County. These services bring healing and strength to people struggling with mental illness.

Crisis Referrals
Same Day Admissions
ReEntry Crisis Residential is co-located with a variety of other services at 1800 Chicago Avenue in Minneapolis, and provides low-barrier access to Crisis Residential Services for individuals residing in Hennepin County.
The ReEntry House Crisis Stabilization Program is located at 1800 Chicago Ave in South Minneapolis. The program is co-located with a variety of other programs intended to support people in need of accessing services. This program provides residential crisis stabilization to adults who are experiencing a mental health crisis.

RADIAS Health strives to make mental health services available to all individuals. In addition to billing Medicaid plans, we partner with local Counties, the State, and Health Plans to make services accessible. At intake, we will work with you to identify and understand any costs associated with your care.
