Community Foundations
Community Foundations provides residential treatment to people struggling with acute symptoms. The program is licensed to serve sixteen individuals, and is located in Maplewood, MN.
Community Foundations is an Intensive Residential Treatment Services (IRTS) program located in Maplewood, MN. Staff provide recovery-oriented, evidence-based treatment and rehabilitative services for sixteen people struggling with acute symptoms of mental illness and/or co-occurring substance use disorders. Each person has their own bedroom and bathroom; this allows for privacy, dignity, and supports the person in providing a peaceful recovery environment.
Services include:
- Individualized assessment
- Illness management
- Integrated services for mental illness and substance use
- Family education
- Crisis assistance
- Development of health care directives and crisis prevention plans
- Nursing services
- Inter-agency case coordination
- Client transition and discharge planning
Specific services and duration of services is based on clinical need. The program has a maximum capacity for sixteen individuals.
