Community Treatment Team
The Community Treatment Team and Extended Care Apartment Program is a unique collaboration with Ramsey County and local property managers. The program supports approximately fifty people per year in maintaining safe and stable housing.
Our Community Treatment Team helps people keep subsidized housing in over fifty different living units within Ramsey County. People served experience barriers including mental illness, substance use concerns, as well as previous struggles in obtaining and keeping a safe and stable home. This program is a collaborative effort with Ramsey County. In order to receive this service, individuals need to have Targeted Case Management services in Ramsey County.
RADIAS Health strives to make mental health services available to all individuals. In addition to billing Medicaid plans, we partner with local Counties, the State, and Health Plans to make services accessible. At intake, we will work with you to identify and understand any costs associated with your care.
