Housing Navigation Program

Housing Navigation services assist individuals who experience a mental illness to find, obtain, and keep stable housing.

RADIAS Health receives Housing Support for Adults with Serious Mental Illness (HSASMI) grant funding from the State of Minnesota. Our Housing Navigation Program staff partner with individuals to find, obtain, and keep stable housing. This includes assistance in accessing housing subsidies and financial assistance resources to make housing affordable. Ongoing support is flexible and based on the needs and desires of the individual served and may include Medicaid funded Housing Stabilization Services.   

In addition, RADIAS Health partners with AEON Housing to provide housing support services to some individuals living in the Crane-Ordway and Renaissance Box apartments.


RADIAS Health strives to make mental health services available to all individuals. In addition to billing Medicaid plans, we partner with local Counties, the State, and Health Plans to make services accessible. At intake, we will work with you to identify and understand any costs associated with your care.
