Targeted Case Management
This program provides person-centered care coordination in areas such as gaining access to medical, social, educational, vocational, and other necessary services as they relate to a person’s mental health needs.
Targeted Case Management provides person-centered care coordination to individuals diagnosed with serious and persistent mental illness who are residents of Ramsey County.
Case Managers help link individuals to needed services, which include mental health, chemical health, medical, social, vocational, legal, educational, and recreational services.
Referrals are accepted through Ramsey County Social Services.

In addition to billing Medicaid plans, we partner with local Counties, the State, and Health Plans to make services accessible.
