Homeless Services

Recovery starts with a safe and stable place to live. Outreach services are intended to engage individuals experiencing homelessness.

Our Homeless Services Programs provide outreach services in Ramsey County. These services are intended to locate and support individuals experiencing homelessness.

RADIAS Health’s ACCESS Team provides outreach services to people experiencing homelessness who have mental illness using federal PATH (Projects for Assistance in Transitioning from Homelessness) funding.

We also collaborate with the Metropolitan Council’s Homeless Action Team, providing housing navigation services to people using transit as shelter. In addition to these programs, RADIAS Health receives some additional state funding to support existing Homeless Outreach as well as engagement with families experiencing unsheltered homelessness. 

Our services are provided by mental health professionals and practitioners trained and experienced in engaging homeless individuals who struggle with mental health disorders and/or substance use disorders. Professionals and practitioners support individuals in accessing appropriate services and resources. Our staff members are highly visible in the community, visiting homeless shelters, drop in centers, and engaging in outreach activities in streets and skyway areas. RADIAS Health Homeless Services staff understand the needs of people experiencing homelessness.

RADIAS Health strives to make mental health services available to all individuals. In addition to billing Medicaid plans, we partner with local Counties, the State, and Health Plans to make services accessible. At intake, we will work with you to identify and understand any costs associated with your care.
