Waivered Services Case Management

Waivered Services Case Management provides assistance for adults with disabilities who receive CADI (Community Access for Disability Inclusion) Waivers in finding and coordinating care with community-based service providers.

The goal of Waivered Services Case Management is to assist individuals living with disabilities in accessing services through CADI (Community Access for Disability Inclusion) Medical Assistance Waivers. Waivered Services are designed to help people, who would otherwise need nursing home level of care, maintain their independence in the community. We currently serve the Hennepin County community. 

Case Managers assist in identifying services, medical equipment, and/or home modifications needed to maintain community-based living in the least restrictive setting possible. Goal development and service planning utilizes person-centered principles and incorporates both what is important to and important for the person.  

Coordinated Services and Supports Plans are developed in collaboration with the person, locating and setting up services with providers in the community, and monitoring the quality of services. Case Managers can also assist with referrals for needed medical, social, educational and other services, regardless of the funding source. 


RADIAS Health strives to make mental health services available to all individuals. In addition to billing Medicaid plans, we partner with local Counties, the State, and Health Plans to make services accessible. At intake, we will work with you to identify and understand any costs associated with your care.
