Targeted Case Management – Hennepin County
This program provides person-centered care coordination in areas such as gaining access to medical, social, educational, vocational, and other necessary services as they relate to a person’s mental health needs.
Targeted Case Management provides person-centered care coordination to individuals diagnosed with serious and persistent mental illness who are residents of Hennepin County.
Case Managers help link individuals to needed services, which include mental health, chemical health, medical, social, vocational, legal, educational, and recreational services.

Our Central Access Team coordinates our referrals for all of our programs. You will be contacted within 24 business hours to determine next steps. We are happy to consult prior to referral.
